how do they kill amp noise?

I was listening to Nirvana's Nevermind, which has very distorted guitar riffs.
What do they to avoid the amp noise that comes from those huge gain settings?
The only thing that occurs to me is gating. Is there anything else?
I guess you'll have to drive the amp's input pretty high, so you have a better s/n ratio. Or is it the other way?
Men, how do the guys of Slipknot or Rammstein get rid of that massive noise?

Noise Gates...gotta have em with high gain tube amps...

I don't use a noise gate in my guitar rig.. but I'll add one as a plug-in in Sonar. This way you can play with different gate settings when you mix and not have to re-record guitars.
 
I sometimes use a behringer FeedBack Destroyer and in cases
where I mic a git amp with fuzz and distortion,I gate the sh#t out of any unwanted noise or feedback. Both in unison can negate
most unwanted signals.
