Miking something "off-axis" means that the microphone is not pointing directly at the sound source - i.e. it is at an angle. Like in this pic, the mic is not pointing directly at this douchebag's mouth. Instead, it is angled downward. That is, "off-axis" from his mouth.

Well off axis is off axis no matter how you look at it. If the mic is cylindrical in shape, and on axis is pointing straight at the source....


So you would read a polar pattern chart as "0" is facing directly to the diaphram. Then you hear things like "45" degrees off axis... so if you look at that same chart, it will tell you your EQ response in relation to your mic placement.
