Tascam 424 MKIII panning problem

No matter what I do, I can not pan my recordings right. I have tried many different settings, but the sound consistently only comes through the left headphone/monitor and fades to silence as you pan right. Any suggestions?
 
Have you tried listening through both the headphones and speakers? If you've tried speakers, which jacks are you running out of? The monitor out or the line out? Have you tried both?
 
I have tried using both headphones and the monitor out. I'll try the line out next chance I get..

Do that ... and then if you could also ... take a picture of the unit with the settings as they are when you're having this problem. If you have to take a few different close-ups to make sure everything is legible, that would help too.

Any particular areas you want close up? I'll probably do this over the weekend.

I think if you just divide the front into four quadrants and take close-ups of each, that should do it.
