How do I avoid "clicks" between CD tracks?

I'm mixing in ProTools. After I "bounce to disk" as a .wav, I add some reverb and do some trimming in Sound Forge. Then when I record an audio CD, I get clicks between the tracks. If I forward to the next track rather than just letting it play through, I don't hear the click. I've tried putting fade-in and fade-out at the beginning and end of the tracks in Sound Forge. This seems to lessen it sometimes, but not always and never totally removing the click. What do I do? -Dave
 
You've mentioned all your software you use, except the cd-burning software. Mostly these kind of problems are being caused because of that. What do you use?
 
I'm using MyCD which is what came with my HP CD-W 9510i. How do I use a different software? -Dave
 
That sounds not too good.

You can buy WinOnCd, it's really cheap and really good.
I've got no clicks or something when using it.
