Hi, this is a bit of a shot in the dark, but I found a thread you started way back in 2009 about an Otari MX5050 that had dead RW & FF buttons except for a 10 minute window where it worked perfectly. I just acquired the same model and have the exact same issue.

Im dragging up a very old post, but........I see that you were to receive an Otari CB-116 Locator manual. I found a partial copy but it is missing pages 2-1, 2-4, 3-2, 3-3, 4-1, 5-1, 6-1 at least. Could you post those pages?
