Microphone placement

I've got a Shure SM57, and a Sutdio Projects B1...

I'd like to know how to place them on...

an acoustic guitar,
a single speaker amplifier for electric clean or distorted,
a cabinet (half stack) for electric clean or distorted,
on a piano (not a grand piano, but an upright),
and a human voice...

Which would be the preferred mic for each and how might I go about placing them best... the piano and voice REALLY concerns me. I don't want to hear a pressing of the keys and I was thinking of pointing the Shure 57 downward for the vocal with a pop filter at about 45 degree angle. I'm concerned with how many inches away I should place it... a small guide came with the Shure mic, is this accurate?
 
Come on man, I'm a newb and even I know that all this information can be easily found in the two sticky threads at the top of the mic forum. If you haven't read these threads in full then you really should spend some time on them. There is a wealth of information there.
