I think melodyne can do that...I don't have the software in front of me, but if my memory serves me right, it has a midi export feature... don't expect it to be perfect, but I've found that melodyne does a good job of figuring out what the notes, etc are in a performance.

reatune/reaper can do it but i don't know how well it works. haven't tried it myself.
