Headphone monitoring without a mixer???

I have a Delta 1010 with an HHB 40 and am looking to run headphones for monitoring. When I plug headphones in the back of the Delta (out 1&2) only one speaker of the headphones work. Do I need a 2 to 1 quarter inch adapter? Or a headphone amp? With headphone amps, I've noticed that they usually have one quarter inch out as well. Help!!!
 
ime not to up in the delta 1010 but you might want to check if the outputs are balenced or not
