Copyright Forms...

I'm having some trouble locating SR and PA forms for U.S. Copyright.... they're around my house somewhere, but I forgot where I put 'em.
Does anybody have any on file that I can print a few copies from?... or even the proper web address where I can get it?
I know it's a government website (dot.gov), but I forgot the address.

WOT THE *$#@&*!!???

I CAN'T GET THE FORMS!!!my fancy f**king computer @ work says it cannot locate adobe acrobats!!!!!arrrghhhh!!!what do i do now?

if you go to the library of congress website i believe there is a 1-800 number you can call and place an order & they will just snail mail you the forms. if you don't mind waiting for a couple of weeks...

there is the the number for ordering forms, where to call
if you need help with which forms to order, and i think they can even fax you a couple of forms if need be...

Online is definitely the best way to go, but you can also go to your local library.
