Cable for Live Wire Solutions HA04

I just bought a Live Wire Solutions HA04 headphone amplifier and I was wondering what kind of cable I need to run from the HA04 to my interface (M-Audio Pro). Not expecting it to work, I tried plugging in my guitar cable and it only plays out of one ear. I would really appreciate it if someone could recommend a cable that will fix this problem. Thanks.

Is your interface the Fast Track Pro?

If you go from the line outputs of the interface use what is normally called an insert cable.

If you go from the headphone output use a 1/4" TRS to 1/4" TRS cable.
