    They are trying to explain to you that "warmth" is a catch all phrase used by marketing guys and reviewers. If you want less presence in the high end, what some would call warmth, your best bet is probably a dynamic or ribbon mic. An SM58 can be very warm compared to a typical condensor.

    That would most likely be done at mixdown. A good example of reverb done in mastering would be the Pearl Jam - Ten album. And a good example of remixing would be when they re-mixed the album recently to get rid of the damn reverb that was done in mastering.

    Philosophy aside, limiting and compression are absolutely neccessary for a modern sound. Luckily you can do it on the computer for a fraction of the cost of a hardware unit. It's not quite as good as great hardware but it willl get you 90% of the way there.

    Unless you are recording cricket farts I can't imagine mic self-noise actually being an issue. Preamps usually generate much more noise then the mics. In most mixes it isn't an issue. If you are trying to record very quiet sources then a good preamp is a must.

    how often do you layer a mix?

    Sometimes it makes sense to EQ on a buss. I'll do it on guitars or vocals but it's usually just for controlling the low end of that part relative to the bass and drums. I rarely do any additive EQ or anything too drastic on the buss level.
