What would cause the phantom power on a console to fail?
What should I be looking for once I get the console opened for repair?

There may be an internal fuse that protects the phantom from shorts, the phantom may have also got a load spike from plugging in a mic with the phantom on which could blow a fuse. If the console has a separate power supply the fuse may be there.
