Problems Recording From PV16FX Mixer to Garage Band

When I record to Garage Band via the USB on my mixer the results are distorted and useless. I just want to record my band and be able to hear it without having to record to a USB. Any suggestions how I can get an audible recording from the mixer directly to my Macbook Pro/Garage Band.
 
What are you recording? (Microphones, insturments, etc) How does it sound when you monitor with headphones plugged into the mixer?
