Peaking glitch (Digidesign 192)

We've encountered a bug which results in channel 2 output continually peaking (although sometimes others in addition) on the Digidesign 192 sound card (channel 26 on the desk Yamaha O2R96), but when the channel is monitored there is no audible signal on the channel.

The only way to subsequently stop the peaking channel is to close the pro tools session then re-open it.

The way i've found to recreate this problem is to open the i/o preferences, change the output of a path, save changes then exit the io settings. Nine out of ten times this will cause the problem, although sometimes it seems to be happening 'randomly'.

I suspect it's a software bug relating to PT10.3.4 and mountain lion.

If any of you pro tools experts have any insight feel free to share!

I'm not aware of this specific bug, but the first thing to do would be to update to 10.3.6.
 
Sorry just noticed that I wrote 10.3.4, however I meant 10.3.7.

Have you trashed the ProTools preferences?
I know that sounds a bit "turn it off and on again", but it is my go-to tool for any odd behaviour.
 
Put in the trash...Deleted.

Have a google; Someone made a nice little app that does it all for you.
Protools pref and database trasher, IIRC.
 
Thanks, it just might be the problem because it keeps crashing more and more often with time
