"Muffled/Shellshock" effect...

I was at the movies the other night watching "Silent Hill". (It was okay...the ending sucked and left too much open...even if there is a sequel coming.) Anyway, during one of the scenes the character gets knocked down or something and the audio turned muffled like it was what she should be hearing. This is done fairly often and it got me wondering, how is this done? Typically in these scenes there will be breathing and a heart pounding, but how is the muffled effect created?
