I want outputs 1&2 to go to the Monitors, and outputs 3&4 to go to headphones...HOW?

I want outputs 1&2 to go to the Monitors, and outputs 3&4 to go to headphones...HOW?

I got an MAudio Delta 66. I want analog outputs 1 & 2 to go to my monitors, and outputs 3 & 4 to go to a headphone amp. I want the same exact mix to go to both. How do I do this in CuBase SX?
 
Setup another output bus using 3 & 4, then use the track sends to create your headphone mix to that output bus.
 
Ok. I created another bus and used the sends to send all those tracks to the headphone bus. However, the panning doesn't seem to translate at all. I want the same exact mix to come out of the headphones that is coming out of the monitors.
