I think Steenamaroo hit the spot. The input's messed up somewhere. It looks like you're recording two things (vocals and piano) on one side of the same stereo track and one (just piano) on the other side.

For some weird reason, it works like this:
From the top menu select Tracks > Add New > Audio Track
It will paste a new empty mono track on the screen (or stereo if that's what you selected). Do additional setting up as required.
