    When you plug a passive bass or guitar straight into a mixer's line inputs you can end up with an impedence mismatch which results in a lot of high frequencies getting rolled off, try plugging into a DI or even the guitar processor set to bypass.

    I've had a drummer bring his own in-ears, naturally they worked fine. The issue is that people don't usually share in-ears you use your own or at least your own molds for hygenic reasons.

    Low frequency hiss ??? Hiss is high frequency and it's pretty normal for guitar amps to put out some hiss. Low frequency noise is usually described as either hum or rumble, which one do you got? Is your mic cable running near to your AC cords?

    Make sure everybody in the band is prepared and knows their parts inside out and upside down and can play them properly blindfolded and in their sleep, because 'take 10' probably isnt going to be that great even if it is correct.

    The Samson looks like a line level device to me, you're probably gonna need seperate amps. Why not just run the phones off an auxillary send with a cheap headphone amp. You can just use an ordinary mic for talkback if you have a spare channel.

    Guitar Fx processors expect to see an instrument level signal, basically the tiny voltage put out by a guitar pickup, The aux sends on a mixer operate at line level which is quite a bit hotter.
