is the output distorted even if you are using only single channel input and channel meter reads far away from 0? Is there maybe heavy compression/eq applied? Does it sound distorted in headphones too?

Thank you for your interest in helping me. We always use the same settings for live presentations. I checked the compression/eq settings and seem to be ok. I restore the mixer to the factory defaults and the output sound is distorted still.

Always using the same settings? Maybe your gain staging is wrong. Make sure that the OL (overload) led's don't light up. If they do turn down the trim. Check with headphones!
