Repairing Headphone Cable Jack

I think this is a simple question to answer but I can't find instructions anywhere.
I have an old headphone extension cable(male and female 1/4" jacks, coiled, about five feet long when not being pulled) that needs a new male jack. The overall diameter of the cable is about 3/16" and inside there are three individually insulated wires(red, black and white).
What kind of jack do I need and how do I wire it? Thanks.
 
you need a 1/4 stereo plug. tip is red, ring is white, sleeve is black
