Does anyone know where to download a multitrack recording form?

Are you talking like a track sheet thing? I just finished doing a quick one with a set up for the Fostex mr16. I can email it Monday when I am at the office. Or do a google search for track sheets and look for a page Or try these links. Mine is similar, but just more Fostex specific. There is also a page with a sheet form made for the Tascam 2488.
