M Audio Fast Track Headphones Only Playing Mono/Won't Pan/Stays in the center

Monitors play in stereo fine but my headphones through my headphone jack plays only mono! Help!!

Im using a 1/4" to 1/8" trs converter jack not sure how to know if its balanced or not.. maybe that could be the problem?

First check that the headphone do in fact get a stereo signal form something else when you plug into it (preferably wiht no adapter).
When you say 'only mono' do mean on one side only or that the same sound is coming out of each side?
Are you sure that 1/4"-1/8" adapter is TRS (3 conductors) on both the plug and jack?

Tip and ring only = no good for stereo.
Tip ring and sleeve will carry stereo.

The screen shot talks about balanced/unbalanced.
Different implications - same jacks.
