Shred is not Dead by Terry Sydek
I studied with him at national guitar workshop, and he's an even better teacher then a player.
You can get the book with dvd at guitar centers, and it's an amazing book. Trust me, get it.
Runner up is Paul Gilbert's Terrifying Guitar Trip (and i've seen many vids from Petrucci, Johnson, etc.)
Rumor has it the Frank Gambale is good
