    Do you have it directly connected to the computer, or to a USB hub? A USB hub can wreak havoc on interfaces. I'd definitely say to get more memory. Check the latency setting in the Tascam ASIO software. Adjust it up and down, keeping in mind what the original was in case you need to put it...

    ALWAYS plug and unplug with phantom power OFF. When you turn it off, give it about another 30 seconds before you unplug the cable just to make sure the voltage has bled off.

    It's not impossible that something has happened. I would say that something has been connected/disconnected at some point while the phantom power was on and possibly damaged something. Just a guess. I've got a US-1641 and haven't had any problems with it.

    No problem at all. It will export .wav files, via the on screen menu. You can then connect to a computer via USB and copy the files from the "WAV" directory.

    A dirty pinch roller (the rubber wheel) can cause some slipping, but I don't think it would cause the tape to come off the roller. Now a bad pinch roller would do that, if it is hardening. I had one or two do that. Replaced the roller and all was good. Just a couple of things to look into.
