Are any of your tracks or busses set to "Interleave mono"? If so, change then to "Interleave Stereo" (obviously LOL)
Look for these icons on the tracks/busses in the console.

Also, your Sync and Caching should be set up similar to this. If all else fails, you might be able to use the "Manual Offset" to sync it up. But that's just a band-aid for the real problem, not the solution.
Whenever I get any hiccups or delays, I usually resolve them by increasing the MIDI playback buffer. I know you had mentioned that you messed with the buffers, but maybe you missed this one.
I've got mine set a little high right now, because I've got several active tracks and libraries running.

Check this out. It might be a better (and cheaper) solution than using the Teensy board. If it's still not what you're looking for, then I can do a quick write-up on using buttons on the Teensy board. It's pretty simple, especially if you only have a single button.
