Guitar to midi

I remember some years ago I had this little free program that would convert the notes I played on my guitar to a midi file the best it could and most of the time it was right on but its been years and I can't find the install file. I'm pretty sure the name of the program was Guitar To Midi and I've googled it with no luck. Anyone know what i'm talking about?

If not, can anyone recommend a good program that would convert single notes played on a guitar to a midi file? I guess its kind of like audio to midi.
