Press/Promotion Pack?

Hi all,

I'm trying to put together a package to give to clubs and venues and was wondering if anyone here has made one before and could give me a good idea of what should go into one.

So far I figure photos, any reviews or press we've received and our first album and a demo of recent material.

Anything else?
 
Making a press kit:
What to include
-CD/Demo
-Business card
-Cover letter
-Head shot/Group Shot
-Bio
-Reviews (most current on top)
-Any press

I've got one last question that is somewhat relevant.

The former lead singer of our band died in 2003. There was considerable press about that. Our group is specifically mentioned and talked about in those articles. Should we include those articles as well? I don't want it to look like we're using his death to get gigs!

Maybe just quotes?

If the articles about your singer's death said complimentary things about your band, why not just pull some quotes from them and use them in something else, like your bio?
 
Upping an oldie to ask some stupid questions....

I'm guessing the bio's are self written, Are they usually in 1st or 3rd person?
What exactly does a cover letter include?
