Does this exist (DI / Headphone amp)

I'm looking for a piece of equipment that will do the following:

- Convert a headphone output to two balanced outputs

- Convert a balanced line output to a headphone output.

Does this exist? I've spliced some schematics and could build one myself. I'm just hoping somebody makes one!
 
The SMPRO M-Patch2 might be what you're looking for.

It has a pair of balanced inputs, and 3.5mm aux input that you could plug a headphone signal into.

It also has two balanced outputs, and you can switch between the inputs/outputs.

This device would need two independent inputs and outputs.

I would be using this box in conjunction with my digital piano, snake, and recording interface.

Signal Chain 1: Piano stereo Headphone output -> TRS Headphone Jack input on the box -> L/R Balanced XLR outputs on the box -> Snake -> Recording Interface.

Signal Chain 2: Recording Interface balanced XLR Line outputs -> Snake -> XLR L/R Balanced inputs on the box -> Stereo Headphone Output on the box.

The closest thing I have been able to find is Level Converting Box.

I figure I could build my own such box for less than $100 and I can't find anything that will do completely what I need for anything less that is in the form factor I am looking for.
