    No, it was a known issue with at least some of them, and it was something that I heard about by the mid-80s when the mixers were probably less than a decade old. Other mixers that suffered the same treatment didn't have that problem.

    I'm pretty sure that back to back cardioid mics, with one inverted, will produce a good figure-8 pattern. That's essentially what a dual diaphragm condenser does.

    Aim the speaker at your face. If it's not pointing right at your head, you aren't hearing the actual tone coming from the speaker.
