recording forms

I'm starting to record some other bands now. And I have a few forms I've made up on the computer. But I was wondering if there is a site where I can find some premade forms. Maybe I'll see something I left off that I should have on mine. Thanks

What fields do you have on the form?

I have:
  • Name
  • Volume
  • Pan
  • Effects
  • MIDI/Audio
  • Changes done...

I have tracks, instruments,who what when where. audio, midi, effect. I have a log for the play back through the mixer and the settings used. Settings on the mixer during recording, (EQ, pan, atten, which mic used ect) I would like some kind of price sheet, and I guess I'm not sure just what I'm looking for. I may have what I need but I'm wondering if I missed anything.

I've downloaded some forms from the net a long time ago (don't have the links anymore), and they basically consisted of 24 tracks, and each track had information about what it was, volume, pan, effects. Nothing more...

Does it feel like you're missing something?
I can't think of anything...
