See what you can do at a place that does printing..You can desige one to your likeing.ie just a grid type or something with space for more info{recall type of sheet also}..Good luck

I don't know any websites, but I have one that I designed for my studio. I actually tried to send you a copy of it, but it looks like you have your e-mail blocked from the forum.
