I spent some (tedious) time at the Library of Congress website, and if I understand things rightly, then an SR copyright covers both the rights to the song and the performance of it. Yes?

Second question: in addition to the CD, do you have to send in the lyrics/chords/melody on paper as well?

Form SR does give you the performance rights if you're the author. Performance rights are automatically vested with the person who created the music. However, once the song/music is first published (released for sale, lease or gift to members of the public), anyone can perform a song (recording or live) so long as they pay either mechanical royalties (for recordings) or performance royalties (paid by the venue/theatre/bar via their license - performers don't have to worry about this one).

Also, Yes, you should still send in the printed lyrics with the Form SR. By the way, you'll need to send in two copies of the CD/cassette with Form SR. Form PA requires only one copy of the song(s).
