Form PA Title Space

Never filled one out, Do I put the titles of the songs and then the title of the work or title of the work and then the titles of the songs? (3 songs) My girlfriend is watching and shes really ready to go to bed, She's the one filling out the PA form. I need to file this in the morn.

In the 'Title of This Work' space, put the name of the album, then in brackets right after describe it as: (an album of three songs).

Then in Section C of the Continuation Sheet list the names of the three songs under this general heading: 'Song Titles'.
