a short sonata

I read some piano sheet music awhile back and I ended up turning this song into an arpeggio/sonata. Does anybody know the original name... Its not moonlight sonata.

Its a pop tune called The Anniversary waltz, written for the Guy Lombardo Orchestra in the 30's.
