Forms used in th studio

I am just about to complete my studio and was hoping that some of the more seasoned members could help. I am looking for examples of any type of forms that would be used in the studio. From the actual contract to forms to indicate instrument/rack placement.

Anything would be more than I'm starting with, so I'll take any suggestions that you might have. MS Word, PDF's or links to locatins that would hav those types of documents would be appreciated

On the VS Planet download section you can find some nice track sheets and forms... They are meant for the VS-series, but the idea and organization is nice!

I gave up trying to attach some of the files we have to this message, seemingly only graphic images are allowed. Anyway, email me at midiguy732@hotmail.com and I'll send you some of our stuff "unloaded". I have a word document that is our track sheet, a spreadsheet which has our patch bay designations, and I think I still have the spreadsheet with all the midi assignments of all the gear.
