What to use for sound monitoring?

Not having a dedicated room I am forced to use earphones most of the time. Nevertheless it is impossible to do without good monitors. So
1. What earphones are the best in terms of price/quality?
2. What monitoring approach and what hardware is best in terms of price/quality? (Active monitors, passive with amp, existing hi-fi, something else.)
