what to put on the back of a cd

Hi,
I was looking at the back of several cd's to see what should be written on ours.
ex. Written by, 2005, etc.
what exactly is the c and p with a circle around it? Is that copywright?

Is there a site or can anyone tell me what information is supposed to be on the cd? I know it varies, but I want a basic idea.

4. Produced by?
5. Some cool photos or design.

I think the P in the circle is about who owns the recording as opposed to the songs?

Also consider including:

artists website address

artists email

a bar code if you think you might sell in outlets where they'll use it (eg a record store).
