stupid darn dumb adat

My dumb 1-8 adat ate my tape i had to take it apart to get the tape out amazingly the tape although looking beat, played flawlessly on the lower adat so it's not the tape. Anyone know why it eats tapes and howit may be fixed??

I have used Ampex and TDK SVHS tapes, and so far I have not had any problems.... just a few weeks with the ADAT....is it too soon to tell? I have also just purchased a JVC SVHS
tape and I will be testing it out soon. I really appreciate all the information on this forum!
