It actually a SM 85, not to be confused with a 58.

I have a SM 85 that need a new grille. I couldn't find a NOS replacement grill, but Shure has a grill for the SM 86 and 87A which are there current SM series handheld condensers. I believe the grill for the 87A is exactly the same and it cost about $40, but the grill for the SM 86 is only $15...
I thought that it wasn't a good idea, which is why I always turn off phantom power first. I think that you end up shorting the +48v to ground (just like you said), which is bad for the supply and preamp since it can send that 48v to the preamp while most pre's can only take up to 10v without...
I have heard patching with phantom power on is not good for the phantom power supply and the preamps on the console and can eventually lead to failure.
How bad is this really for you pre's and has anyone had experience with increased noise in the pre's from this or complete failure?
There are some firewire consoles that work as audio interfaces too.

They typically get called patch cables when they are shorter in length, and with narrower connectors and for use in a patchbay or patching something on the back of a board, but they don't have to be used for those purposes.
