Placement of foam paneling for vocals

If I use foam paneling to deaden the room for recording vocals, is it enough to cover the front and sides, or would I have to cover above and behind, also?

Put enough up to where it sounds good. If you don't hear any room reflections on the vocal tracks I would say you are done. If you do hear reflections then throw some more around the room.
